Bilingual Spanish Case Manager -Life is Precious Location this is a teen suicide prevention program
Location: 3245 Hunters Point Long Island, City 11101
Hours: 10:30AM to 7:30PM- Required to work Saturdays
Responsibilities:
- Completing comprehensive assessments, service plans, progress notes, monitoring and re-assessing client’s needs for treatment and community education on chemical dependency related issues.
- Develop supportive relationships with clients; link them to community based medical and mental health care
- Conduct comprehensive assessments and service plans; provide crisis intervention to clients as needed; assist clients with accessing primary medical care and other necessary services to increase their capacity for independent living; provide and coordinate supportive services as needed; monitor clients’ progress and provide individual and group workshops
Qualifications:
- Bachelor’s Degree in human services or related field; Credentialed Alcohol and Substance Abuse Counseling (CASAC-T or CASAC) is a plus
- Two (2) years’ work experience
- Experience with HIV/AIDS afflicted individuals, chronic mental illness, substance abuse individuals and their families a plus
- Must be bilingual Spanish
