**Social Worker (LMSW level) Needed for ER Department**
ASAP to ongoing – 37.5 hrs a week Mon-Fri only 9a-5p

Job Description:The Social Worker will be working at the ED department doing psychosocial assessment for at risk patients being dischrged and addmitted to the hospital from the E.R..Will work closely with ED RN Case Manager to prevent avoidable admissions, re-admissions and to transfer patients to in-patient psychiatric facility as indicated. The SW is tasked to call patient after discharge within 24-48 hours or as needed. Will also be providing in-service Education to ED Staff on specific issues as requested (Domestic violence, child abuse, elder abuse. etc.).
REQUIREMENTS: Must be an LMSW in the state of NY, with at 2-3 years of related experience. Must have good follow up and organization skills. With great computer skills and EMR experience.
