**Medical Social Worker (LMSW level) Needed**

Job Description:The SW will be part of a multidisciplinary team working with patients and their families and caregivers to assess patient and family psychosocial and discharge planning needs relevant to medical treatment. The will provide intervention, emotional support, resource information, discharge planning and legal reporting.
REQUIREMENTS:Must be an LMSW in the state of NY, with at least 3 yrs of Social Work experience and ideally minimum of two years experience in discharge panning in acute hospital/inpatient setting.
