Currently looking for Case Managers to work in Manhattan and Brooklyn.
- Candidates must be willing to work with the homeless and substance abuse population.
- Complete service plan reviews and other clinical assessments.
- Write progress notes, assist with maintaining clinical charts.
- Carry a caseload, conduct home visits, maintain entitlements, escort clients, and liaise with outside providers.
Bachelors Degree is required in related field. 1-2 years prior experience is required.
